Ambulatory care or outpatient care is medical care provided by Sublime Nursing, on an outpatient basis, including, observation, consultation, treatment, intervention, and rehabilitation services. The purpose of providing our clients with ambulatory care is to prevent or reduce the need for hospital admission (or inpatient care), such as diabetes or chronic obstructive pulmonary disease.
Every patient travels comfortably from the hospital to their home when they have the assistance of an experienced nurse or who always puts patients’ health and safety first. Sublime Nursing’s healthcare professionals will help patients settle into their homes, when they are well enough to be discharged, but need further support and personal assistance once they get back to their own home environment. Our nurses can accommodate any personal or medical needs, from changing catheters to wound care, to picking up medications. At Sublime Nursing, our skilled nurses are specialized in elder, adult and mother/infant care are available with the option to extend outpatient services. A companion will assist with any non-medical needs, errands and activities, including but not limited to food shopping, prescription pick up, dry cleaning pick up/drop off, attending other social services.
